Gynecomastia

Gynecomastia or Gynaecomastia is the abnormal development of large mammary glands in males resulting in breast enlargement. The condition can occur physiologically in neonates(due to female hormones from the mother), in adolescence, and in the elderly (Both in adolescence and elderly it is an abnormal condition associated with disease or metabolic disorders). In adolescent boys the condition is often a source of distress, but for the large majority of boys whose pubescent gynecomastia is not due to obesity, the breast development shrinks or disappears within a couple of years.

The causes of common gynecomastia remain uncertain, although it has generally been attributed to an imbalance of sex hormones or the tissue responsiveness to them; a root cause is rarely determined for individual cases. The condition may also be caused by an imbalance of estrogenic and androgenic effects on the breast, resulting in an increased or unopposed estrogen action on breast tissue. Approximately 4 to 10% cases of gynecomastia are due to drugs. Aldactone (spironolactone) is a very common culprit that has a 17% rate of developing the condition. Both Digoxin and Furosemide are also reported to cause the same condition. Although, chances of gynecomastia could be more if these two drugs are co-administered. The condition known as pseudogynecomastia, or lipomastia, is characterized by fat deposition without glandular proliferation. Although gynecomastia is usually bilateral, it can be unilateral.

Male Breast Reduction

Male breast reduction (also known as gynecomastia) is the surgical correction of over-developed or enlarged breasts in men.During an excision procedure, the incision is made either on the edge of the nipple (areola) or in the underarm area. The surgeon cuts the excess glandular tissues,skin and fats from around the pigmented area surrounding the areola and from the sides and bottom of the breast.
Liposuction is also used to remove excess fat, a hollow tube (cannula), is inserted directly through the existing incisions. The cannula is attached to a vacuum pump to suction out the fat.

Duration of the operation:
The procedure takes about 1-2 hours

Post operative care:
To help reduce swelling, you'll probably need to wear an elastic pressure garment continuously for 1-2 weeks, and for a few weeks longer at night. Although the worst of the swelling will subside in the first few weeks, it may be three months or more before the final results of your surgery are apparent.You'll be encouraged to begin walking around on the day of surgery, and to return to work when you feel well enough, which could be in a couple of days. Stitches are removed 1-2 weeks after the procedure.Your surgeon may advise you to avoid sexual activity for a week or two and heavy exercise for about three weeks. You will be told to stay away from any sport or job that risks a blow to the chest area for at least four weeks. Your activities should be back to normal after about a month.

Recovery period:
Depending on the extent of surgery, patients are generally able to work after 5-7 days.Bruising and swelling will gradually subside for several days and more strenous activities can be done after 2-3 weeks.

Possible risk and complications:
Infection, fluid accumulations, injury to the skin, rippling or bagginess of skin, and asymmetry. Pigment changes (possibly permanent if exposed to the sun). Excessive scarring if tissue was cut away, and need for a following procedure to remove addition tissue.

Anesthesia use:
The surgery requires general or local anesthesia with sedation

Hospital admission:
This procedure may be performed in the cosmetic surgeon’s office-based facility, an outpatient surgery center, or at a hospital. This procedure is usually done on an outpatient basis

Preparation before Surgery

• Assessment of the medical history (any allergies, serious medical condition and all medications taken both prescribed and non-prescribed), physical examination, and laboratory tests will be performed during consultation.
  Blood and urine samples will be collected for routine preoperative laboratory tests.
• Smoking must be avoided for about 3-4 weeks prior to surgery, as nicotine interferes with circulation and will greatly affect healing process.
• You will likely to be asked to stop drinking alcohol, a week before the surgery and throughout your recovery period.
• Avoid taking any medications such as hormones, anticoagulants, anabolic steroids and supplements at least 4-6 weeks to prevent complicating medical factors prior to surgery. Avoid taking aspirin, anti-inflammatory drugs and herbal supplements as they can increase bleeding.
